AgeSong, a locally managed, privately owned group of senior living communities with an integrative approach to care, has partnered with San Francisco based Hampton Health Ltd., under the medical direction of Dr. John H. Fullerton, to provide assisted-living residents with a more comprehensive geriatric care and wellness program that truly integrates the psychological, social, bio-medical and spiritual aspects of care. Through this partnership, AgeSong becomes the first San Francisco based assisted living provider to offer an on-site medical office and physician services.
“Incorporating a medical office inside our AgeSong communities in San Francisco enables us to utilize innovative methods, and deliver in-depth, comprehensive and compassionate care to our residents,” said Nader Shabahangi, Ph.D., founder and CEO of AgeSong. “To respond to the increasing high-acuity needs of our residents, we are bringing top experts in geriatrics that share our vision for integrated care that addresses the physical, emotional and psychological needs of elders.”
The new collaboration is a milestone towards optimizing the health and quality of life of older adults at AgeSong communities. Founded by the internationally recognized expert in geriatrics, John H. Fullerton, M.D., The Hampton Health On-Site Geriatric Medical Practice is appointing Dr. Theresa Kemnitz as the On-Site Physician and Associate Medical Director at AgeSong.
“As elders become more frail and forgetful, taking them into the hospital environment poses certain challenges,” said Dr. Kemnitz. “By bringing personalized, accessible, on-site geriatric care and consultation, we can reduce the unnecessary transfer and trauma of hospital visits, greatly enhance safety, and enable the staff to address any change in condition of an elder.” Dr. Kemnitz has specialized in the musculoskeletal system designed to address the needs of preventive and holistic medicine and has an extensive experience in the field of long term care.
As part of their ongoing medical care, the Hampton Health Ltd. team will support the coordination of residents’ medications, diet needs and physical therapy. They will work closely with caregivers, health care providers and AgeSong staff to help integrate all aspects of the residents’ care. They will also offer basic treatments and preventive measures such as flu shots, B12 injections and drug administration for diabetic elders. Together, Dr. Fullerton and Dr. Kemnitz will work closely to ensure the best possible care for the AgeSong residents.

Dr. Fullerton’s distinguished career includes numerous awards and recognitions. His work and current credentials include clinical faculty positions at UCSF, Stanford Medical School, and Yale University Medical School. He serves as Education Director of Geriatric Medicine for the internal medicine residents at St. Mary’s Medical Center in San Francisco.
